New Appointment Credit Card Reservation Payment
A deposit of $150.00 (one hundred and fifty dollars) is required to reserve your new patient appointment.
You will loose your deposit if you fail to cancel your appointment with at least 2 business days notice. Upon arrival to the office your deposit can be used to pay your out of pocket expenses (copay or coinsurance) or "no show" fees. Any remaining deposit will be kept on your account for one year or until it is used for out of pocket expenses or no show fees. No show fees are currently $150.00 for new appointments, $50.00 for special tests or therapy, or routine follow up appointments.
If you decide to cancel your appointment you must personally call the office and obtain a cancellation code to validate your refund request. Cancellations can not be made by fax, email or any other form of communication. You may request a letter or email with your cancellation code. Your cancellation must be made at least with 2 business days in advance. For example if your appointment is on Monday at 2pm you must call no later than the previous Thursday at 1pm to cancel your appointment.
