Information for New Patients We thank you for choosing us to provide your neurological care. It is important that you review the information below before your first appointment. These guidelines will assist in providing you a more expedient visit and a comfortable experience for you and the other visitors as well. Please don't hesitate to call us with any questions.

- Please arrive 30 minutes before your appointment if you have not filled your forms in advance. You can download and print the forms below and fill them prior to your office visit, then you will not need to arrive in advance to your appointment.
- We have one office in Katy. We closed the Spring Branch/Houston office. Make sure you got the right directions for the right office.
- You will need to bring the following:
- A government issued picture identification such as a drivers license, state id, military id, passport, etc
- Your insurance card
- Your copay or coinsurance whichever applies. For your convenience we accept all major credit cards, checks and cash.
- Please bring any records or radiological studies pertaining to your visit, if any have been done.
- Only one adult companion, over the age of 16, per patient is allowed. Unattended children under 16 may NOT be left in the waiting room. We regret that we can not provide childcare services.
- We serve patients of diverse cultural backgrounds, so please come dressed appropriately: No short shorts or mini skirts. No see-through clothes or extreme decolletage. No sleeveless shirts. Wear shoes at all times while in the office.
- If you have medical records, copies of tests, or films, related to your current medical problem please bring them with you. If you have radiological studies (MRI, CT Scan) we prefer that you bring the film since we are not always able to open discs (CD, DVD) in the office.
